ESNEFT Community Services
North East Essex Community Services aim to serve our local population needs, close to home, promoting health equality, and help prevent unnecessary admissions to hospital. We support around 15,000 unique patients each month, often with chronic, multiple and/or complex health needs, or approaching end of life. Due to this complex nature, we have a central passion to deliver holistic and integrated support, working closely with our acute care, primary care, social care and voluntary sector partners.
Services Include:
- Community nursing teams, supporting diabetes, heart failure, incontinence, tissue viability and palliative care.
- Integrated Community allied health professional (AHP) teams, including physiotherapy, occupational therapy, speech and language therapy, dietetics, podiatry and prosthetists/orthotists.
- Specialist support for long-term conditions, offering services for people with acquired brain injury, neurological conditions, diabetes, heart failure and COPD.
- Urgent and emergency response care to support admission avoidance.

Who can use this service?
NEE Community Services supports Residents under a North East Essex GP Practice. Please seek advice from your GP on appropriate services and referrals required to support you.
How are services delivered?
Depending on individual needs, patients may receive care in Community Hospitals, GP practice clinics, their homes, or in care homes.
