North Colchester PCN accepts referrals, via Frontline, to their social prescribing service from ECC Adult Social Care teams. Patients must be registered at one of the following surgeries: Bluebell Surgery, Highwoods Surgery, Lawford Surgery, Mill Road Surgery or Winstree Medical Practice.
